Participants are requested to register in advance either by online registration or by hardcopy registration form, which should be faxed or mailed to the Registration Desk. Note: No registration is complete until the full registration fee has been received.
Early/Pre registration opens: October 20, 2008
Deadline for early/pre registration: April 20, 2009
On-site registration is available at the venue.
Registration Fees (Japanese Yen)
The registration fee differs according to the category of the participant and the registration date.
The fees listed below are given in Japanese Yen.

*1 Members of JCS receive a discount in recognition of their host country financial support.
*2 Co-medical participants are required to send a written certificate from their immediate superior to the registration desk by fax or post to prove their status as a co-medical. Co-medical category includes nurses, X-ray technicians, pharmacists, physical therapists. Doctors can not register in this category.
*3 Student participants are required to send a copy of their student ID to the registration desk by fax or post.
*4 Abstract co-authors can not be allowed to register as an accompanying person. Please note that accompanying persons are only family members or spouses of registered participants.
Accompanying persons can not participate in any of the scientific sessions.

Payment Procedure
Payment Method
All payments must be made in Japanese yen by bank transfer or credit card. No other type of payment will be accepted.
Deadline for registration by bank transfer: April 20, 2009
After April 21, 2009, only registration using a credit card is accepted.

Please note that all related bank charges must be paid by the registant.
The bank transfer information will be on the last screen of the on-line registration. Alternatively, this can be sent to you by email after completing the online registration process.

For quick and accurate processing of your registration, please email or fax a copy of the bank remittance receipt.
Credit card
American Express, Visa, MasterCard, Diners Club and JCB are accepted.
Confirmation Letter
Your confirmation letter and receipt can be printed from the Congress website after the application is completed and payment received. Please remember to bring your confirmation letter with you when you sign in and pick up your materials upon arrival at the Congress venue.
Cancellation Policy
The Congress will refund registration fees to prepaid participants who cancel their registration, with the amount of the refund based on the date of cancellation.
For cancellations made before March 18, 2009, a handling fee of 20% of the registration fee will be deducted from the refund. For cancellations made on or before April 18, 2009, 50% of the registration fee will be deducted from the refund. For cancellations made after April 18, 2009, no refund will be given.
