^*2	Fellows of APSC are persons who became a Fellow of the Asian Pacific Society of Cardiology in December 2007. All Fellows must send a copy of their Fellowship certificate to the registration desk by fax, post or e-mail. Please note that Fellowship status will be confirmed by the registration desk.
^*3	Co-medical category includes nurses, X-ray technicians, pharmacists, physical therapists and others working for a medical institution, but not a person working for a commercial supplier in the industry. Doctors can not register in this category. An official certificate or a letter signed by one's immediate superior must be sent to the registration desk by fax, post or e-mail as attached document.
^*4	Graduate Student/Undergraduate Student must send a copy of the student ID to the registration desk by fax, post or e-mail as attached document. Resident/Trainee must send an official certificate or a letter signed by one's immediate superior to the registration desk by fax, post or e-mail as attached document.
^*5	Abstract co-authors can not be allowed to register as an accompanying person. Please note that accompanying persons are only family members or spouses of registered participants. Accompanying persons can not participate in any of the scientific sessions.
^*6	Date: 21 May, 2009 Time: 19:30-21:00 (Tentative) Style: Buffet Note: Gala dinner can be applied and paid with on-line registration system. Gala Dinner has limited number of seats. First come first served. Therefore, ticket could be sold-out before the congress dates. Payment Procedure

Payment Method

All payments must be made in Japanese yen by bank transfer or credit card. No other type of payment will be accepted.

Deadline for registration by bank transfer: April 20, 2009

Bank transfer (telegraphic transfer only)

Please note that all related bank charges must be paid by the registrants.
The bank transfer information will be on the last screen of the on-line registration. Alternatively, this can be sent to you by e-mail after completing the online registration process.
*Overseas remittance: Please indicate the participant's name and application number issued as the last step of the online registration process) in the message field on the remittance application.
*Domestic remittance: Please indicate the participant's name in the sender's field of the remittance application.
If you are unable to provide the information as above, please contact the registration desk.
For quick and accurate processing of your registration, please e-mail or fax a copy of the bank remittance receipt.

Confirmation Letter

The registration confirmation and the payment receipt can be printed immediately after completing the Online Registration. Please see the instructions for "HOW TO PRINT" below.

<Paid by Bank Transfer>

After your payment has been confirmed by the registration desk, a confirmation email will be sent to you and then you can print the registration confirmation and the payment receipt directly from the Congress website. Please see the instructions for "HOW TO PRINT" below. Please note that it can take about a week to confirm the payment in some case.

HOW TO PRINT

^2.	Enter Your ID and password.
^3.	Click "Confirmation"
^4.	Click "Print a confirmation letter" or "Print a receipt" to print out.
*	Confirmation can be printed as many times as you want at your convenience by May 16, 2009.
*	A receipt can only be printed once. Please inquire at the registration desk if you failed to print.

Cancellation Policy

The Congress will refund registration fees to prepaid participants who cancel their registration, with the amount of the refund based on the date of cancellation.
For cancellations made before March 18, 2009, a handling fee of 20% of the registration fee will be deducted from the refund. For cancellations made on or before April 18, 2009, 50% of the registration fee will be deducted from the refund. For cancellations made after April 18, 2009, no refund will be given.
